To engage services with us, we follow a straightforward three-step process:

Meet and Greet
Contact us to arrange a meet and greet. During this initial meeting, we want to get to know you and ensure that you have a voice in your care. We’ll discuss your goals, interests, hobbies, and past care experiences. What worked well? What can we improve? How do you want your services delivered? Your input is invaluable in shaping your care.
two.
Collaborate and Creating Your Care
Using the information gathered in our initial contact, along with advice from your healthcare providers, and in alignment with the National Disability Insurance Scheme (NDIS) or the Aged Care Assessment Team (ACAT), we will draft a Service Agreement and Plan for your review. This plan reflects your identified care needs and is flexible, allowing for edits and adjustments to ensure it accurately meets your support needs and can grow with you.
three.
Match and Engage Support
We will find a suitably matched nurse and/or carer based on skill set, location, availability, and personality traits. Once matched, services can commence. We also offer the option to schedule regular meetings—weekly, fortnightly, monthly, or every three months—to provide progress updates, seek feedback, and make any necessary adjustments to your care plan.
We endeavor to find you a suitably matched nurse and/or carer, taking into consideration skill set, location, carer availability and personality traits and preferences and you are ready to commence services. From here we can schedule billable meetings (at the frequency of your choice) which can be formal or casual setting catch ups, to provide you with updates on progress, seek any feedback and provide you with any recommendations for your future and together, make changes to care plan as required.
